NEW: Resource Facilitation Program RFP (Currently covering metro Atlanta counties for qualified individuals impacted by a Traumatic Brain Injury TBI).
As part of your rehabilitation and journey, we offer follow-up service coordination for individuals impacted by a TBI whether newly injured or post TBI.  This confidential and voluntary service DOES NOT REPLACE any medical or rehabilitation follow-up care that you may need. It is intended to provide you and your family with a two year person centered service coordination approach, information about a TBI and assistance in accessing services and support so you can live in your home or community. Complete the form and if you have any questions or would like an appointment via Zoom or FaceTime (iPhone), contact 404-556-2528 or email info@braininjurygeorgia.org Subject Line: RFP inquiry
